In the blink of an eye, our year in Hawaii has come to a close. I am extremely grateful for this amazing experience and for the opportunity to live in such a unique place. I’d like to take a moment to thank a few people. Your support, encouragement, kindness, laughter and sense of camaraderie made this an amazing experience. I’ve been one month in SoCal, one month in NOLA since October. It’s been exhausting, but still fun to get to have all my favorite things in my life. Can’t believe this chapter of residency in my life is coming to a close soon. It’s been an arduous yet fulfilling three-year journey with moments and experiences that I’ll cherish forever. I couldn’t have asked for better residents, faculty, and mentors to work with and help me grow as an individual. As always, I’m thankful and blessed to have the support of my family and friends- I wouldn’t be where I am today without them. But as one chapter closes, another one begins. I look forward to building on what I’ve learnt in residency and continue to improve each and every day as a physician. It’s been a long haul, but worth it all to earn the privilege of caring for the sick in their moments of crisis. I turned in my pager today, and so closed the door on Internal Medicine residency. I owe an immeasurable debt to my wife Erika and my parents Ron and Jo for shouldering the burden and encouraging me on the journey here. “You’re still working?!?!” *insert shocked face. ________ It’s typically the 3rd or 4th question in the line up when people talk to me about my treatment/cancer. And yes, I still work full time. It’s always shocking to people because having cancer and getting chemotherapy tends to not fall in line with working. _________ But, I say the same thing every single time: I absolutely love my job. I love being a PA and helping my patients. I love being able to bring even a little bit of hope & happiness to a patient who’s having to be stuck in the hospital because they’re sick. Trust me guys, I know the feeling because I was just in that (very uncomfortable) bed not too long ago. Plus it’s a great distraction. So why wouldn’t I work if I can? _________ Today I got a reminder of one of the other biggest reasons I love my job & why I’m blessed to work with my group: a special delivery of these huge gift baskets filled with so many incredible goodies to help make this portion of my journey more comfortable along with some envelopes with other goodies to help ease the burden financially — all from my amazing work family. _________ They are hands down the most amazing group of doctors and PAs/NPs I could have ever asked to work with. It’s truly a blessing to be surrounded by a work family who is encouraging, supportive, and makes you laugh on a daily basis. Oh and actually encourages you to use your PTO when you need/want to. I’m 100% certain that God knew exactly what he was doing when he led my path to work with the Hospitalist group here at Sacred Heart Hospital; granted, he could have made that path getting here a bit less rocky but nevertheless I’m thankful to be a part of the team. ________ Thank you everyone! ♥️

Thank you @sarahreckmd for inviting me to share my goals as I really needed to do this. Thank you for inviting me since it really is so important for us to set goals, write them down so that we can more successfully achieve them and stay focused. Thank you @nataliecrawfordmd for getting this movement started. Sharing them on this platform is taking it to another level (in a good way). My goals on this platform are the following, not in any particular order or written in any particular way except as they come from the heart. 1. To empower physicians of all genders to share their stories about life in medicine. I hope that we can share our struggles, challenges and tell stories mainly of success but also bring to light that the road to becoming a doctor is not a straight or easy one. We all have our story and we are also human. 2. To be a part of the physician community that wants to maintain integrity in the patient-physician relationship. We have been labeled “providers” and become lost between endless hours spent documenting, administrative tasks and have allowed insurance companies to take over and decide what our value is and how much time we should spend with our patients. 3. To empower women and discuss issues that we share as #workingmoms and since I am one, particularly #physicianmoms. 4. To educate and empower others to take control of their health and wellness. 5. To provide mentorship and motivation for #premed#medstudents and #residents. 6. A hospital is voluntary for the most part so if you want to leave and you are of sound mind, you can leave, even if it’s not in your best medical interest. Unfortunately, far too many people leave the hospital “AMA” (against medical advice”). But when they then come right back same day or next day, it’s like, huh? I thought you didn’t like it here — why u back man?? 😳😬They have all kinds of reasons, and of course we admit them right back into the hospital even though they were hostile. This is an enormous waste of resources, involving an unnecessary ER visit, reestablishing treatment plans that shouldn’t have been interrupted, etc. Articles have been written about best ways to accommodate someone wanting to leave AMA, and there’s all kind of liability concerns. Congrats to my husband on winning Resident Teacher of the Year !💥 . But this award got me thinking 🤔 . Cuz I’m a deep thinker and thought stimulator as my bio says ⬆️ . One is considered to be successful when he or she wins an award🥇 . Or perhaps when they earn a certain amount of money per year. . Or when they purchase that dream home and park a fancy car 🚗 in the driveway. . And while my husband by all standards, has and could have such things, what he has taught me, is that success is defined by what actions an individual takes on a daily basis, when no one is watching or keeping score or giving out awards. . Success is defined by a man who has never called out sick with the exception of ONE TIME in almost 30 years-because he has a work ethic stronger than anyone I have come across. . Success is defined by a man who shows up to his office every day, and cares deeply about the under-served population he desperately tries to fix-despite the completely limited resources they have and the lack of wherewithal to know what’s best for them. . Success is defined by a man who works TWO JOBS in order to keep his family financially secure with college-age kids, goals of savings, holding up child support and keeping his demanding wife happy 🙋🏼♀️while she pursues her dream career. . Success is defined by a man who wakes up at 5 am every single morning to get his workout done for the day, packs a healthy lunch and tells his partner she was the most beautiful woman at the event we attended last night 💑 . Success is defined by a man who strives to be the best father possible, by working on himself in order to serve the absolute best interest of his kids, admits his mistakes to them, teaches them that even adults need help to, and that’s ok 👌🏻 . And success is leading a group of young, eager doctors 👩🏼⚕️ in a way that inspires them to love medicine, want to be better physicians and move to change the current state of our ailing health care system for the better. . Today was day 13 of 13 days of work in a row. I am exhausted. I pick up my kids from their dad tomorrow. Today was also the perfect example of how I make it all work. I outsource. A lot. I had my groceries delivered by a delivery service and they showed up at my door shortly after I got home from work. My housekeepers cleaned my house today. My house was clean when I got home. I received my box of clothes styled for me every three months. The box of clothes was waiting for me at my front door when I got home. Last night I got together with a couple of my #hospitalist friends. As we discussed our days, dreams and more, I couldn’t help but feel incredibly lucky and grateful to be a #hospitalist#physician. I completed a four-year residency in #internalmedicine and #pediatrics at Duke University Medical Center and also earned an #MPH while in medical school. Since then I have continued to get additional training in LEAN, biostatistics and more as I consider myself a lifelong learner. I have always loved reading, writing and #clinical medicine. Going into hospital medicine allowed me to follow these pursuits but to also expand my interests into #patientsafety and #quality while delving into #HIT and healthcare innovations , even building up my own health #tech company @valeet.healthcare . At the same time, I get to work with residents, work on policy related issues on a local and national level. I get to be an advocate for patients and for physicians too. I have diagnosed and treated conditions ranging from TTP to brain cancer. So far, my career has allowed me to evolve, grow, learn and thrive. Hospital medicine gives flexibility and options! There are times when working for a hospital can have its challenges with all of the changes in healthcare so those thinking of entering this profession may want to consider. But I believe where there’s a will there’s a way. As an administrator, clinician, teacher, mentor, innovator and more, I feel like my background as a hospitalist can bring a lot to the table in any discussion related to healthcare. And the best part, in my opinion, is connecting with so many bright, talented, passionate and #pioneering colleagues like the #doctors standing right next to me. If you’ve ever been in an ICU or PCU/step down unit, I’m sure you’ve heard the distinct huffing and puffing noises from the #BiPAP machines in the hallways. Gain enough experience and you can even tell if someone is failing Non-invasive positive pressure ventilation (#NIPPV) just from the sounds! Throughout the next few days I’ll be going through the data behind why us ICU docs love this piece of technology so much! This #cochranereview (usually the end-all be-all’s of meta analyses) goes over why we have started slapping these bad boys on the face of every #COPD exacerbation with a pH of less than 7.35 and a PCO2 greater than 49. A 🎩 tip to the many authors of all the studies that were included in this. #myspecialtystillrocks Hi! I’m an Internal Medicine (IM) physician and one of the questions I still get asked a *lot* is, “What are you going to specialize in?” Most people (I’d say 99%) don’t know IM is a *specialty*. The assumption is that if you’re an Internist/IM physician, you haven’t “specialized” yet - not true!! IM is one of the most dynamic fields there is. You can do inpatient (work in a hospital) or outpatient (work in a clinic). Some people do a hybrid of these, where they work inpatient and outpatient. My husband @mfanees has a good description for what an Internist does: they’re like the quarter back of the team, they co-ordinate, brainstorm, manage a variety of clinical situations. When needed, they seek input and assistance from sub-specialists like Cardiologists, Hematologists or from other specialties like Surgery or Psychiatry. I plan to work as a Hospitalist (inpatient), here’s what I love about it: 1️⃣ Fast paced medicine: there’s so much to figure out about patients’ illnesses in a short period of time. There’s no sitting around - it’s all fast action and helping people get the right treatment. 2️⃣Work/life balance: most Hospitalists work 7 days, 12 hr days, then off 7 days to recover, which helps mitigate burnout. 3️⃣We got your back! IM physicians manage most of what a patient needs when ill in the hospital - chest pain, abdominal pain, stroke, seizures, kidney disease, diabetes complications. We appreciate our colleagues from other specialties but enjoy being our patients’ primary doctor in the hospital. 4️⃣Love and respect from patients and families: Nothing tops this. The heartfelt “thank you” from families just makes my day. I love discussing the basis of their health condition so they walk out with more knowledge and feel they made informed decisions. 5️⃣Multidisciplinary work model: IM coordinates not just with other physicians but with nurses, dietitians, speech and physical therapists, case managers, rehab facilities - to get patients the care they need. American College of Physicians issued breast cancer screening guidelines for average-risk women. While these guidelines are written for physicians I think all women will find this information useful! 🔶In average-risk women aged 40-49 years, clinicians should discuss if screening for breast cancer with mammography is right for them. Discussion should include potential benefits and harms and preferences. One should keep in mind the potential harms often outweigh the benefits in most women in this age group. 🔶In average-risk women aged 50 to 74 years, clinicians should offer biennial mammography. 🔶In average-risk women aged 75 years or older or with a life expectancy of 10 years or less breast cancer screening should be discontinued. 🔶 In average-risk women of all ages, clinical breast exam to screen for breast cancer should not be used. Happy Tuesday! As an Internal Medicine Hospitalist, I take care of sick patients and coordinate their care in the hospital. I also see a wide variety of patients and perform a good amount of procedures at the bedside. Despite these benefits, there are also challenges. 🥼 Hospitalists’ schedules may disrupt work-life balance: Most Hospitalists work 7 days on and 7 days off. By choice, I work 7 days but I may not always get 7 days off due to my other administrative and Pre Operative Clinic duties. I don’t always have 7 days to recuperate and recharge. In addition to long stretches, some of the shifts are 12 hours long. I have missed a child’s sporting event or play, the occasional family reunion and even Thanksgiving dinner. 🥼 Hospitalist work is emotionally demanding: I have to break the bad news about the newly found cancer. I care for cancer patients who have complications from chemo, who are on comfort care, and who are imminently dying. It is emotionally taxing for the patient, their family and me. It is devastating and it never gets easier. 🥼 Hospitalists can’t always solve every problem: At times, the treatment plan is on hold and we have to wait for the recommendations from the specialists or surgeons. When they are in clinic or in the operating room, they are not able to come and see the patient until the very end of the day. This may be disappointing to the patient and Hospitalist. 🥼 Hospitalists may seem to provide fragmented care: After working 7 days, we hand off our patients to the next Hospitalist. Some patients feel frustrated when they need to retell their story and get used to a doctor with a different style of care and communication. This affects a patient’s trust and downgrades their overall experience in the hospital. But if you really know your patient, show empathy, and actively listen, you can prevent this. 🥼 There are pros and cons for every specialty in medicine. In my case, #myspecialtystillrocks. For me, nothing will ever feel more rewarding than taking care of patients during some of the hardest times of their lives. Have you ever had diarrhea while taking antibiotics? For most people this is mild and usually does not require treatment as it will clear up within a few days after you stop the antibiotic. Less commonly a more serious diarrhea can occur caused by C. difficile bacterium. It is thought to occur when the antibiotics upset the balance of bacteria in the digestive system and allow C. difficile to quickly grow and attach the lining of the intestines. Symptoms of lower abdominal pain, cramping, fever, nausea/vomiting, loss of appetite, and blood in stool. Had my first experience being part of a second victim case. Medical errors are prone to happen- no matter how meticulous you are. Some will be your fault and some you’ll have to take the fall for. Unfortunately, I had a case full of medical errors this week. From the admission to the medications to the overall management and I blindly walked into it. Studies have shown that the second victim is health care providers involved in an unanticipated medical error & can result in long lasting emotional trauma. We spend energy & resources with patients and their families trying to recover from such events but the providers often suffer in silence. It's important to acknowledge these moments, ask for support, and learn from the situation. Another piece to think about is why these medical errors occurred? Are we getting the help we need? 19.6.17 저는 입원전담전문의를 오랫동안 하고 싶습니다. 그러기 위해서는 야간 당직 근무가 가장 큰 장애물입니다. 야간 당직 근무에 따른 체력적인 소모·불면증·우울감 등 다양한 문제가 올 수 있기 때문입니다. 그래서 야간 당직 근무가 있는 주는 전날부터 수면 시간을 조절하고 먹는 식사 종류의 조절 등 제 나름대로의 루틴(routine)을 가지고 근무하고 있습니다. 3년간 시행착오를 겪으며 만든 루틴이지만 정말 제 생각대로 오래 유지할 수 있을지는 확신을 가지고 이야기 하기는 아직 어렵습니다. 이런 고민은 저만 가지고 있는 것은 아닙니다. 통계적으로도 전국 내·외과계 입원전담전문의 124명 중 야간 당직 근무를 하는 전문의는 채 20명이 되지 않습니다. #입원전담전문의#호스피탈리스트#hospitalist#야간근무#의협신문
It’s one of the most used, least-contemplated words in medicine: baseline. “Baseline” may be used to describe how a patient entered our care - but it isn’t the same as “healthy.” Who do we serve when we comment on a patient’s “baseline”? In medicine, what are our priorities when we shoot for baseline, but no further? Does baseline medicine have a purpose or does it stifle intellectual curiosity and lower the standards of our care?
